Is pain in the upper abdomen along with constipation and diarrhea a serious issue?

For over a year I have had pain in my mid-left back that spread to my left upper abdomen, just under my ribcage/heart. For about 4 months now I have also been having alternating constipation and diarrhea with a very foul smell (the diarrhea is only once or twice a day before I get constipated again for several days). Since then I have also been getting nauseas on and off and have a decreased appetite most of the time, but no vomiting. I have lost almost 10 pounds in the last 3 weeks. I feel very fatigued, and my pain is only getting worse. It is usually a deep cramping, but at times it gets so sharp that it s hard to breathe until it subsides after a few moments. I went to the ER and they referred me to a gastro, she recommended an endoscopy but I can t afford it. Does this sound like something serious? If it s just IBS I don t want to ask family for money to get tests done, but if it could be serious I need to find help to pay for the testing.

Hello and Welcome to ‘Ask A Doctor’ service. I have reviewed your query and here is my advice.

* As per my surgical experience, this may indicate underlying intestine issue of vivid presentation as inflammation, malignancy or others.
* I strongly recommend to consult an expert and take necessary CT scan, colonoscopy to find out the issue, as the normal upper GI endoscopy does not rule out all the possible causes.

Hope I have answered your query. Let me know if I can assist you further.

For the past month I have had changes in bowel movements, sometimes normal, sometimes constipation, or loose stools. I also seem to have indigestion with belching at times and increased gas. I feel bloated and uncomfortable but not any sharp pain. Discomfort is both upper and lower abdomen. Symptoms seem worse if I delay eating or eat a smaller meal. Full meals seem to relieve symptoms temporarily. I have had similar issues before but this seems to be lasting longer. I have tried calcium chews and Protonix for a few days. Gastrict noises in the lower abdomen are very noticeable when I go to bed along with gas. Symptoms seemed to have gotten worse after 10 days of amoxicillin for a sinus infections about a month ago. I am 66 years old. I take 50 mg of metoprolol, 160 mg or Diovan, 10 mg or citalopram daily, along with 600 mg Calcium supplement, Vitamin D3 1000 units, and a multi vitamin all prescribed by my cardiologist who also implanted a pacemaker for sick sinus syndrome 2.5 years ago. Otherwise I am in good health overall.

I am currently being tested for IBS, as I have had severe bouts of constipation and diarrhea for well over a year. I have occasional blood, pencil thin, loose stools and external hemorrhoids. I am also experiencing a constant pain in my upper abdomen, coupled with severe nausea and a feeling that I am going to get sick. I had to submit a stool sample today, and I noticed that there were a lot of dark green, leafy looking substances in the stool. They were tough to the touch but tore apart when pulled. I haven t eaten any sort of green vegetable in well over a week. Any thoughts?
