hi,doc! my daughter of 3yrs 10mths,15kgs is always having a problem of cough n cold..it starts with runing nose followed by hacking cough and at times wheezing also is present.the cough is more at night in lying down position..the paediatrician says it is allergic bronchitis . we have done skin prick allergic tests ,tested for around 83 allergens..she is more allergic to inhaled items through nose like dust, pollens, mites, fungi ,cobwebs...etc doctor put her on bricarnyl syrup, sinarest syrup,montair- 4 tablets for 4mths, nasonex nasal spray and seroflo inhaler..i have given this treatment in july 2013 when she had this problem.she improved after a long time and now since december 20th 2013 she started with the same symptoms and he has given the same medications as earlier.. but the cough is very bad at night and more in lying down position..so, the doctor added today omnacortil 5mg/5ml syrup,bd for 5days..with nebulisation with asthalin every 6-8hrly... doc ! am worried that at this age only so much of medicines ,will bricarnyl, seroflo inhaler n nasonex nasal spray in long term have any side effects on my child? is omnacortil 5mg safe? please advise,am very worried... thanks so much in advance...

Pediatrician 's  Response
Hi

Cough more at night or early mornings suggests there is a wheeze.
Wheeze happens when there is narrowing of the airways. Narrowing of the airways occurs due to spasm of the small muscles in the bronchus and also due to lot of inflammation.

Medications like bricanyl act on the small muscles and help in opening up of the airways. The inflammation that happens can be tackled only with steroids- which is given as inhalation (seroflo etc) or orally (omnacortil).

Since the episodes are frequent, medications like Montair or Seroflo are given to prevent such episodes. They are given for a minimum period of 3 months. Depending on the response they are given for 6 months to 1 year also.

When ever medications like Seroflo are given, the technique should be right. It should be given with a spacer and mask and get the technique checked by your doctor.

For the present episode please follow your doctor's advice. Nebulization and omnacortil for 5 days will help and are safe. Please speak to your doctor and restart her on Seroflo in the right technique. If it was the winters that have aggravated her symptoms, you may give it till the chill is gone.
Minimum of 3 months is needed.
You may also speak to your doctor regarding the use of Asthalin/ Levolin through the same spacer and mask than a nebulizer. It is much easier than nebulizer.

If its the cough that is bothersome and not the running nose, the nasal spray can be stopped after discussion with your Doctor.

If possible try to avoid exposure to triggering factors like cold air, sudden change of temperature, strong smells from perfumes/ agarbathis, smoke of any sort, house moulds, dust, pollen, fur toys, carpets, pet animals, cockroaches etc and what ever allergy detected on allergy tests.

If Seroflo is given correctly for appropriate duration, there will be no need to give the other medications like bricanyl, asthalin often.

Hope this answers your query.
Best wishes.
