Yes there can be blood inside the eardrum when there is injury to the head and fracture of the temporal bone. Or else, it is difficult to have blood inside the eardrum.

The ear drum is a known as TYMPANIC MEMBRANE, can get perforated, as its a memebrane (like a cover/shield) separating the inner ear from the outer ear. No, clot cant be IN the membrane. If there is bleeding or blood is seen around it, then ENT specialist should be seen immediatley.
