Is A Follicle Measuring 28mm On Left Ovary Normal In Size?

I am on cycle day 6 of a typical 36 day cycle. Yesterday on cycle day 5 (period just finished) I had a transvaginal ultrasound. They advised I had a dominant follicle measuring 28mm on the left ovary which halls a volume of 18ml. I ve been trying to get pregnant for 6 cycles now without any positive test. Is there a problem with my follicle size? What can I do?

General & Family Physician 's  Response
Hi,
Thanks for posting in HCM.
I understand your concern.
A follicle basically looks like a small bubble which is about 3 to 4 mm in size. It is present on the surface of the ovary and contains 'oocyte' (egg) in follicular fluid.
This follicle will normally grow to 18-24 mm size prior to rupture.
Ultrasound can predict the approximate time of ovulation and determine whether or not the follicle has truly ruptured and released the egg for fertilization.
The size what you have mentioned seems that the follicle has grown a little larger in size at a early stage. Sometimes, it can happen this way when the follicle is filled more with fluid or when there is no maturation of oocyte in the follicle but filled with fluid only. However, you can follow-up and time it when it ruptures this time.
In case this is happening often, it would be better for you to take ovulation inducing drugs on consultation with Gynecologist.
Hope the information provided would be helpful.
All the best.
