General & Family Physician 's  Response
Hello,
Thank you for your query.

Nolvadex is not advised during breastfeeding unless there is a clear clinical indication .

Lower back pain is a common symptom Associated with many illnesses including cancer (depends ding on where it is).
It does not, however, mean that you have cancer if you have lower back pain.

Please check with your OBGYN if you have any other symptoms like abnormal vaginal bleeding or persistent back pain or family history of cancer.

Do not take any medications without consulting your doctor first, especially if you are still breastfeeding. The stress of feeding and being a new mom can also cause back pain in which case proper support and rest will help.
A urine test can help check for urine infection which can also cause back pain.

Hope this helps and let me know if there is anything else I can help you with.
Wish you good health
