Is HbA1c Level Of 7.2 A Cause For Concern?

My doctor says my kidneys are functioning at 30%. I am 66 yr old female going through chemo for lung cancer (adenocarcinoma) since March. Diabetic for 20 years (A1C = 7.2) . He s given me a high fat diet excluding all sugar, carbs. At what stage is dialysis recommended. What can be done to reverse this kidney issue.

In adult patients without any comorbidities, an HbA1C level of less than 6.5 is desirable. Considering your age and other morbidities, a level of less than 7.5 shouldn't be a concern.
Renal dialysis is usually done when the creatinine clearance dips below 10mg/dl. It can be done earlier depending upon other factors like increasing serum electrolytes or when patient has symptoms of renal failure. You can ask for creatinine clearance values and discuss with your nephrologist regarding dialysis.
Kidney function can be restored if it is an acute injury that is causing the dysfunction. in chronic conditions like diabetic nephropathy, reversal is difficult but can be maintained by taking adequate precautions. Do follow your nephrologist advice on this matter as it is difficult to comment on reversibility without knowing the complete details.
