Thanks for your query. You are having swollen, itchy and tender great toe but the important point is that it is bilateral and for both feet. So this virtually rules out any insect bite, infection or allergic reaction.
Bilateral painful toe swelling is a characteristic feature in gout, venous thrombosis due to blood clot or arthritis. Since you are having arthritis, so this is an important possibility as it can be due to worsening of arthritis. But at the same time, gouty arthritis needs to be ruled out. In gout, uric acid crystals deposits in the joints causing swelling, pain and tenderness. Big toe is commonly affected. Please consult an orthopedician and get your blood uric acid levels and an x-ray of the toe joints done.

i have psoriatic arthritis and 2 years ago I suffered from pulmonary embolisms. Ever since the embolism I have noticed that my right foot is purple on the top close to my toes and sometimes the ball of my foot hurts so bad that I can barely walk. Putting on shoes sometimes is extremely painful and I am worried about my foot. my family doctor says it is vienous pooling and my rhumatolgist says it is the arthritis. any ideas cause my husband is really worried about it and I am tired of being uncomfortable. I have varicose veins as well, and most of my arthritic symptoms are in my knee. however I am sore in my hip and shoulders and hands as well. I take the following medications, warfarin, celexa, synthroid, and naproxen (as needed) also creams for my psoriasis. I was on methotrexate however I stopped taking it because I didn t feel as though it was helping at all. I feel frustrated and tired all the time so any help you can give me would be appreciated. My main question is were do I go next. I have multiple medical issues. My health has gone down to the point where I lost my job and have filed for disability by advice from my primary doctor. On a general question asked before the doctor responded my issues may be mental related, IE all in my head. Somatoform disorder is what he said. But first let me explain every issue I had been diagnosed with I saw specialists and everything was confirmed on tests. I do have active ulcerative colitis via 17 biopsies. I do have active psoriatic arthritis although my form effects more of my major joints than fingers and toes. I do have arthritis and degenerative disks disease in my spine. My arthritis in my spine is worse in my neck. All my life since being a child I have experienced myoclonic jerks. They can come in clusters in the day but mainly happen when I try to sleep. As a child I had my fair share of head injuries. One requiring blood removal from my skull at age 10. I have experienced blank out spells as a child that got me into a lot of trouble at school and at home. My primary doctor back then did not do any tests or send me to see a specialist. Instead I was put on ADHD drugs. I have noticed multiple times what some call auras. I can be watching TV when suddenly everything in the room looks far away and suddenly all sound goes silent. 5 years ago my GI doctor was trying to figure out why my colitis was not responding to medications other than steroids. An endoscopy found a tumor and multiple tests and CT scans later a trauma surgeon took my case and did the surgery. After the surgery my colitis started getting better but my arthritis exploded. I was diagnosed with spondylosis in my neck with multiple collapsed disks. C1 threw C3 are the worst. I began having stronger myoclonic type attacks in the evening before bed and in the morning. My mother would even tell me I was jerking and blanking out during the day for which I had no memory. Also a few back I started having double beart pause heart beats. Sometimes instead of returning to normal beats i would go into tack for a short period. Then suddenly I started getting these slow motion attacks as I tried to sleep a few weeks ago. They would start with an uplifting feeling in my gut that would spread into my chest. All sound would go away as I felt a slow motion electrical type shock hit me that seemed to take minutes rather than seconds. Then suddenly it was over and my heart would be pounding and I would be sweating. The only emotional feeling was scared that it happened wonder what just happened. These attacks happened back to back as the night went along. I ended up not sleeping for 3 nights straight. I contacted my primary and he put me on klonopin 1mg at bedtime and asked me to see a neurologist. The neurologist asked me to see a psychiatrist to rule out panic attacks. Panic attacks were ruled out so i went back to the neurologist. Instead of asking me questions I was chewed out for letting my doctor put me on klonopin or even suggesting I might need an EEG study. I was sent to a sleep study clinic which ruled out sleep apnea and sent me back for an EEG study. My neurologist then gave in and did a 2 hr sleep deprived study which showed nothing despite being told by the tech I had 2 myoclonic type jerks and asked me why I never hit the panic button so they could be identified better. So my neurologist basically pushed me out the door at that point and doesn t want to see me for another year. what bugs me most is that i worked in orthopedics years ago and know that RA in the neck can effect the brain. I also have a family history of ALS and picks disease. What should I do now. Should I seek out a second opinion? If so should this opinion be from another neurologist or a different field like a spine doctor?
Hi, i am writing on behalf of a friend. He had been diagnosed with having IBS when he was 13, without a colonoscopy, and now he is 25. He has been having abdominal pain and interchanges of diarrhea and constipation all these years but a month ago it suddenly got really worse (after having some fish, but even if it was bad, he had never experienced such excruciating pain again), as in one day he started to ache so much he couldn't get up off the floor. It was like stabbing and ripping him apart. He went to the hospital and had blood work done and an X-ray and they said it was normal but suggested a colonoscopy but he didn't go through with it. Ever since he has been having really severe abdominal pain and interchanges of diarrhea and constipation and he has been to several doctors, one prescribing him with amitriptyline and antibiotics for the intestine, and another after that giving him mebeverine hydrochloride alone. He took the antibiotics and amitriptyline for almost a month and it helped him a bit but in the evening he would none the less have the return of the severe pain in his entire abdomen, sometimes centering on the left upper side of it. Then he went to the other doctor who perscribed mebeverine hydrochloride, telling him it would instantly help, without that being the case when he tried it. He had extensive blood work done and everything was normal, no inflammation or anemia, and the doctor said he didn't need anything else and should just take the mebeverine. But he is still in pain and i was wondering if it could be something else with these symptoms. Should he have a colonoscopy or some other less painful test? He also has Gastroesophageal Reflux Disease.
Yes, I have a 13 yr old that has been vomiting and has had constipated/diarrhea for a month now. She has been seen by her physician and was prescribed Prilosec and miralax. She appears to be full of stool ( according to her x-ray). She is still having dull stabbing pain in her stomach and is still vomiting daily. She expresses vomiting is not always with nausea. I hope she will not need an endoscopy. I am wondering if it s GERD or IBS. Thanks for your response.
Dear Dr, I am 30 years Male. I had productive cough in morning for last 2 months and I was suffering from sinusitis also. The sputum was green. Now the cough is gone and only sinusitis remains, no that much sputum. I done a chest x-ray last week which came normal. However doctor told a elevation in Diaphragm and enlagement of heart. What will be the reason of this?. Active Pulmonoray TB not there, but will it be Miliary TB symptom or Amoebic Liver Abscess or anything else?? Please advice me for the next step
Hi there. I am a 35 year old female 5'7 155 lbs with a 7 month old baby delivered by cesarean. I have had shortness of breath that worsens with physical exertion since the last month of my pregnancy but am otherwise in good health. The last 2 months it has been getting progressively worse to the point where I have to catch my breath sometimes in conversation. I am taking domperidone to increase my breastmilk production and have been for 2 months. I have been to the ER where I have had an ECG, ct scan, x-ray and bloodwork, as well as a pulmonary function test. All results were within normal ranges except my oxygen levels in my blood were low - however the test was taken peripherally, not through a central line. Also, my resting heart rate was 118, and when I did a stress test, within 2 minutes my heart rate was at 200. They are sending me for an echocardiogram next. Do you think this could be adult onset asthma or a reaction to domperidone or is it my heart?
