Urologist 's  Response
Hello and welcome to HCM.
As an Urologist, i can fully understand your concern.
Your mother will need maintainence dialysis or a kidney transplant.
Maintainence dialysis, 2-3/week, will maintain her kidney function.
If her age is below 65 years, she should go for a kidney transplant.
Till a transplant is done, her symptoms can be controlled by medicines.
She must take only 5gm salt, 40gm protein, low fat diet daily.
No fresh fruits, and fluids must be restricted to urine output +400ml daily.
Hope your doubts are cleared. Dr.Matthew J. Mangat.

Gastroenterologist Dr. PAVAN KUMAR's  Response
hi
thanks for posting in HCM
I have understood your concern

probable reasons are
1.inadequate dialysis leading to fluid over load
2. high BP induced cardiac dysfunction and pulmonary edema
3. anemia
4.calcium and phosphate abnormality

I can suggest you to get done
1.hemogram to check for anemia
2. serum calcium and phosphate

treatment
1.depending upon swelling of lumbs, breathing difficulty she needs increase in the dialysis
2.better control of BP tab. moxovas 0.3 mg twice daily and tab.arkamin 0.1 mg thrice daily, she needs adjustment depending upon BP levels

regarding transplant
1.she needs her BP control
2.cardiac fitness , if LV function is normal , she can go for transplant.

hope this helps you
any further questions please let me know
thanks
