How to treat a fourth degree shoulder separation?

I have a fourth degree shoulder separation sustained in a fall today and want to locate an orthopaedic surgeon in the Claremont, California area, who could perform surgery asap and at what cost. I have x rays and a video of how the injury was sustained. Please contact me on YYYY@YYYY

You will have to attend an ER to schedule a surgery under emergency. meeting a surgeon in outpatient will not help.
