above describe imaging finding are compatible for chronic pansinusities involving left maxillary, ethmoid and frontal sinuses with possibility of polyp. Blocked osteo-meatal complex secondary to mucosal thickening.

Hi , As per your imaging it appears as you are having Chronic Rhinosinusitis with polyposis.
Start Steam Inhalation , Saline nasal drop , Steroid nasal spray with antihistamine for 1 month , Antibiotic azithromycin & ornidazole for 10 days. After completing above treatment go for 1 month go for FESS ( functional endoscopic sinus surgery) .Continue Steroid nasal spray with antihistamine ,Steam Inhalation & Saline nasal drop after surgery also. This will improve your quality of daily life.
