How to cure sleep apnea?

I take medications that make me very sleepy at work. I also have sleep apnea; both of these conditions make me so sleepy at work that I do fall asleep at my desk (usually no longer than 5-10 minutes) for which I would take my break at that time. There have been times in which I fall asleep without knowing that I fell asleep. Someone told me that there is a form that I can have my physician to fill out that would cover my work accomadiations. I am coming very close to being fired for this and I need something so that I can continue to work. Is there such a form?
Thank-you, Alice
