How To Control Body Weight And TSH Levels During Conception?

My husband and I are planning a second baby. As I was obese and my first pregnancy involved gestational diabetes and hyper tension issues leading to a C section delivery I was keen to get my health in order in advance of the second baby.In October 2011, my weight was 84 kilos. I applied rules of healthy eating and daily walking and successfully brought the weight to 70 kilos by July 2013. At a height of 151 cm this meant a BMI of 31. One of my friends suggested a dietitian as I was already being good about my eating habits (eating moderately, on time, less sugar, less oil, no junk food, etc) with regular walks (about 4-5 days a week) without any weight loss. She suggested blood tests where my TSH problem was first diagnosed (TSH was 4.98 at the time). she prescribed 25mcg Thyronorm and I have been taking it from 1st August last year. She advised next blood test in 5-6 months along with a low calorie diet and moderate exercise plan. I have been good about diet and exercise for the last 5+ months and my weight was 61 kgs around February 9. The journey from 65 to 62 was particularly tough and long and took lot of effort and perseverance. Unfortunately despite being very good about both diet and exercise my weight increased to 63 kgs on February 13 - almost 5 pounds within just 4-5 days.My dietician again suggested a blood test. So last Saturday (about 5+ months after my first TSH, T3, T4 test) I got another blood test done. The latter two are fine. TSH is at 4.55.She advised that I visit an endocrinologist who has increased my dosage to 50 mcg Thyronorm every day for the next 6 weeks. He has also advised that until we bring my TSH levels to 2.5 I should not plan a baby. The doctor was vague about how long this process will take.Aside from weight gain and super difficulty in weight loss, I also have problems sleeping at night, fatigue, constipation and blurry vision concerns for the last few months. I am 34 years old.My question is how will we arrive at the best dosage that will help elevate the symptoms, bring TSH to safe levels, a healthy BMI, so that it's safe for me to conceive? And how much time will this take. At my age and with my history should I go in for a second pregnancy at all.

Thanks for the query. I understand your concern.
You have multiple factors to be checked &keep control on while weight reduction.
-Have a regular follow up investigations & consultation of an endocrinologist to keep thyroid Hormones normal, which not only help controlling body weight but also is essential for fetal development.
-You had gestational diabetes during last pregnancy...please get glucose tollerance test to exclude diabetes. (It may enhance weight gain/cause problem in conception). &start needful medication in case you have raised blood sugar.
-Diet control to be followed with expert chart provided by a fietitian.. aftershowing her all the investigation report.
-Small &several servings of food including salads,vegetables & nutrients of all varities (one by one) in small quantity.would help weight loss.
- Sufficient fibers in diet .. proteins in prescribed quantity in 24 hours.&plenty of water helps good motions .
-When for reducing weight. Physical work outs need to be under physical instructor's guidance
-sound sleep for 6_8 hours mskes lot of difference in weight reduction (please take help of Yoganidra cassette /start meditation)
*Working from all the aspects regularly would give you desired weight with normal hormone content.
In such condition you can go for child bearing under an expert gynecologist's guidance... to overcome all above hurdles with pregnancy in forties. ...for positive results!Wish you all the best!
