He has high BP - currently 170/90-100 Sodium was once as low as 114 also and he had terrible wheezing He is sleeping most of the time and wakes only for approximately 4 to 5 hours. Physical Therapist or Physiotherapist 's  Response
For ischaemia and high BP first have check on his BP regularly. Because most of the time the cause for stroke is BP. If it fluctuate a lot then consult your doctor and change the medication.
For leg swelling get if it's constant apply a crew bandage as well keep it on a hight with the help of pillow below his ankle level and possibly he should try to move his feet and legs as much as he can. If he is not able to move then somebody should do it for him.
For copd cases wheezing is a common problem. He has to improve his respiratory health. Either by doing pranayama or breathing exercise or do Spiro metery exercise regularly. Spiro metery is a small unit available in medical store which help patient to improve respiration.
If there is a chest congestion then looking at his age I suggest to take help of Physiotherapist also to make him do all this things. And do consult with your doctor on a regular basis to have check on his reports and medicine.
Hope this was helpful to you. Take care.
