How Should Bipolar, ADHD And Suicidal Tendencies Be Treated?

My 9 year old daughters been diagnosed with bipolar ADHD...prior to diagnosis shed had PTSD diagnosed due to family separation, divorce and temp move to live with grandfather. Since moving back home with me and her sisters, visitation with father she has been transferred from home school as overflow to another school which her therapist said was not in best interest. School district didn t accommodate her ..she had severe outbursts and distress with school. To this day school is huge issue and problem getting her to cooperate to attend. She threatens self harm...physically has to be carried to school in building. The district tested her my request put her on 504 plan..I feel iep is best. What s your take?

Diagnosis and treatment should be made step by step to avoid multiple psychotrpics and it should be problem based and not for the label
so, I would like to start with the treatment of bipolar. I like to start with Lithium with therapeutic level monitoring
that is lithium level should be maintained between 0.8 to 1.2 preferable (0.8 to 1)
then u would like to treat the patient with non pharmacological measures for ADHD
with these treatment, if there is persisting ADHD symptoms, I would like to add methylphenidate of bipolar is adequately controlled. since there are lots of psychological issues in the background, I would also start her on trauma focused CBT.

This is in accordance with practise parameters given by American academy of child and adolescent psychiatry
