How Should An Old Head Injury With Bruising And Pounding Be Treated?

Two years ago I fell on the edge of a sidewalk and hit the back of my head. I had a cut that was about 4 to 5 inches long. I was so disoriented that I just fell asleep and the next day blood covered my whole head, the mattress and the walls. I didnt get it checked cause I didnt want a doctor to shave my hair. Now 2 years later its healed but it still feel like a bruise and somedays I still feel it pounding. What could be wrong with it and should I spend the money to get it checked?

Hi, thanks for asking from HCM.


I can understand your concern. After 2 years, your injury must be healed, unless there is some pus discharge or CSF discharge. It can occur if there was bone and dura damage, which did not heal. You can get it examined by doctor. If there will be active wound, he will try to rule out

- chronic sinus due to osteomyelitis/ingrowing hair

- CSF leak due to dural and bone damage

If there is doubt, he might ask for NCCT head and/or MRI brain. Further treatment will depend upon diagnosis. Usually both scenarios need active surgical treatment. Excision of infected bone and antibiotic course. In case of CSF leak, repair of dura and bone defect will be needed.

Another more likely possibility is healed hypertrophied scar. It can feel like swelling, appears red/pink and may be tender sometime. But it is a scar (means healed area). There is no need to do anything if no severe pain and it is covered in hair. The feeling of pounding is due to radiation of pain at this area. It is normal to have pain radiation at scar site.
For pain, you can take tablet Acetaminophen or diclofenac after proper prescription.
