How Do I Write A Letter To My Landlord To Allow Raising Pets At Home?

My son has severe ADHD but seems to respond positively to dogs. I have been researching emotional support animals and have decided to talk to my doctor about writing a note to give to my landlord ( as he currently doesn't allow pets in our rental home) that states my son needs the dog. How would I go about asking his doctor and what should the note say?

Thanks!
