Renal function is monitored by maintaining a chart of blood urea and serum creatinine levels.... during each visit of dialysis you must carefully maintain and note down the date, the levels of renal parameters in a book... regularly you must show it to the nephrologist.. This will be use useful to you in case of emergencies, regarding your previous treatment...hope I have answered your question..
