General & Family Physician 's  Response
in my opinion, taking both paracetamol and ibuprofen will not have any significant cross reactions ,
in the information you didn't mention which side of shoulder pain and what kind of pain like dragging or throbbing or aching type. as you told he is working (physical) so might be muscle spasm or ligament tear might be possible reason for it .

measures :
take analgesics (ibuprofen or aceclopenac) ,
give rest /immobilise the shoulder for few days
even light heating with sponge (cloth) will reduce pain

still pain persist , kindly visit orthopedician.

I hope you happy with information, kindly rate it. thank you.
