General & Family Physician 's  Response
Hi,

If anti-allergic (antihistamines) and steroids does not improving urticarial reaction than possibly the allergen is constantly present inside or in contact with the body. There are multiple possibilities which must be rule out.

One of the reason could be autoimmune disease in which antibody targets body's own cells and causes such reaction. So consult allergic specialist to rule out causative allergens and autoimmune disease if any present.

Hope I have answered your query.
