Hi....I have gone through your question and understand your concerns.
He is experiencing Pleuritic chest pain. It can due to pneumonia, pleural effusion (fluid accumulation in pleural cavity), pulmonary embolism, pneumothorax and sometimes due to cancer. As he is old age so the typical feautures of a disease may not be there. I suggest you to take him to your physician to have physical examination and I suggest he should have CXR (chest xray) and If needed than other tests like pleural fluid aspiration or CT scan or d-dimers after CXR.
Meanwhile you can give him analgesics for pain relief.
Hope I have answered your question. If you have any query feel free to ask.
Wish him good health.
