Hello and Welcome to ‘Ask A Doctor’ service. I have reviewed your query and here is my advice.

Nose bleeds are very common, usually due to a fragile vessel being damaged and the blood thinning with warfarin making bleeding difficult to stop. So it may be nothing to worry about. However INR levels are needed in such cases to check for very high INR which would warrant a dose decrease. If an isolated episode and INR is where it should be I would live it at that. If the episode reoccurs though or if there are bleeding phenomena elsewhere in the body then more thorough investigations are needed regarding coagulation, starting with blood count, platelets etc and nasal exam for abnormal blood vessels. As for the "lumps" coming out of his mouth it must have been blood clots from the nose passed from the back of the throat in his mouth, or ingested and then regurgitated, it happens when there is such a severe bleed, not much significant in itself.

Hope I have answered your query. Let me know if I can assist you further.
