How Can My Grandson's Habit Of Passing Stools In His Pants Be Corrected?

Grandson purposely poops his pants, he goes and hids and then poops. he laughs and thinks it is funny. He has told his mother if she puts a potty chair in the family room on the couch and etc yet he is now 6 years old and doesn't do it at school. Parents have tried making him clean it up himself. I know there are problems at home but he is smart and knows what he is doing ? Any suggestions. He is very spoiled and grandparents buy and give him everything.

hi thanks for contacting me here

1.Iam really sorry for your worry, but your question do have the few answers ...family problems, overconcern on child...( i would be able to help you further if you tell me what is the exact problem at home)
2.Overflow incontinence(encoprosis->pooping in inappropriate places) is not unusual problem for a 6 years oldand they hide it beacuse of gulit and to save themselves from verbal abuses from adults
3.Constipation and over flow incontinence go together.May be we have a constipation there which needs to be addressed with proper diet,if needed medications in continuum and a doctor for local examination to rule out anal fisures
4.Diet : lots of fluids, fibre is important as hard stooling can cause a viscious cycle of pressure to pass a stool ->fissures->pain ->retention->overflow incontinence (encoprosis)
5. This is a huge topic to discuss..so to begin with please be seen by a psychologist for counselling purpose for the entire family and not just the child, make few diet changes (lots of fluids (minimum 1 lit/day), fibre in diet ), NO CARBONATED DRINKS, be seen by a pead surgeon to rule out local causes like anal fissures/polys and then we can start the child on Movicol (macrogol) if there is no disimpaction just on maintainence ...
1 sachet per day in 1 glass cold drink daily before bed to produce soft stools
and behavioural training like reward systems for using potty 4-5 times /day each time for 10-15 mins each time and alliase with school nurse to know any history of bullying in school or abuse which your child might be hiding

thats all for now
more information and help if you come back
regards
dr sowmya
