General & Family Physician 's  Response
Hello,

He needs parenteral anti-coagulant (heparin). Based on the history given your husband seems to be having Coumadin drug resistance resulting in recurrent DVT.

I advise him to consult the vascular surgeon for further evaluation and treatment of DVT which includes injection heparin. He also needs to switch over to other oral anticoagulants like Apixaban for the prevention of thrombosis.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Mohammed Taher Ali
