Hello and Welcome to ‘Ask A Doctor’ service.
I have reviewed your query and here is my advice.

In my opinion, it is very important to take a detailed history of alcohol use. Alcohol can cause suspicious NES and aggression too.

It is very important to detoxify him from alcohol use and treatment with benzodiazepines and thiamine.

Later on to de-addiction treatment can be carried out. If suspicious NES is persisting, then treat him with antipsychotics drugs.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Gayathri
