General & Family Physician 's  Response
Hi,

As there is an intense pain at 4 weeks of injury, it suggests that there are disturbances of the natural healing process due to an underlying crack in the bone or dislocation.

Immediate attention with ortho surgeon is warranted.

Primary relief with ice application, analgesic as Tramadol Hydrochloride if no history of drug sensitivity.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Purva Patel,
