General Surgeon 's  Response
Hello and Welcome to ‘Ask A Doctor’ service.
I have reviewed your query and here is my advice.

The need for general anesthesia depends on size and location of lipoma. If this is larger than 8 cm or placed near blood vessels, nerves, and joints then it is much better to do it in general anesthesia due to possible pain and complications during surgery. The ache and itch are common symptoms of deeply located lipoma and this is nothing unusual, this is not a sign of malignancy but in every case, after the removal it should be sent to patohistologic analysis just to be sure about this.

You don’t need to be afraid of the surgery since it is simple and routine, and it will prevent it from further growing.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Ivan R. Rommstein
