How Can HIV Be Prevented After A Possible Exposure?

Today I went to visit an escort. I have visited a few escorts over the past year and always practice safe sex. However today the condom broke while I was having vaginal sex with her and I am very scared about the risk of HIV exposure. Ok so during vaginal sex the condom must have broken as I noticed the condom was like a ring round the bottom of my penis...leaving my tip exposed. Sadly didn t notice this, I am not sure when it broke but we were having sex for 5-10 minutes so it could have been broken for any amount of that time.When I noticed the condom failure I immediately started freaking out...I talked with the lady and she assured me she was clean and gets tested regularly etc etc. My biggest fear is HIV as it is of course incurable and the fact she is a sex worker makes me worry more. I freaked out so much I went straight from the sex workers place to a local sexual health clinic for advice there...I had googled there are drugs (PEP) that can be started within a day or 2 after a potential HIV exposure that reduce the risks. I spoke to a nurse and was told that the drugs are themselves quite damaging to organs etc and that based on my case I didn t warrant them. The nurse told me that you won t have contracted HIV and told me quite confidently - but part of me just thinks this was to make me feel better as I was pretty disheartened and she could tell I was worried during our meeting Of course now I am home I am still freaking out...I essentially had unprotected intercourse with an escort. I wanted another opinion on what the risks are and if it is the right thing not to be starting PEP. The lady was white british, looked visibly healthy and told me she was clean...however she is a sex worker, that is my biggest worry. We also started by using a condom which must show that she is used to operating in this way by doing protected sex

Broken condom in the midst of act cannot be considered as totally risk free. Better to have PEP. People are on ART for more than 15 years without any complications. Nothing will go adverse with drugs.

If infection established it is lifelong problem. So, start PEP not later than 72 hours after the exposure.
