Hi,

Promethazine is not very much known to cause nose bleed. Nasal bleeding can be because of stuffiness, allergic rhinitis. If its large in amount then she should to taken to emergency. If small in amount, she can wait n consult during routine working hours. She can go for X-ray chest and X-ray PNS if repeated episodes occur. Nasal polyps need to be ruled out as well.

Take care. Hope I have answered your question. Let me know if I can assist you further.
