I have gone through your query regarding headache,
nasal obstruction, pain radiating to face and ear. You have also co-related your headache with periods which indicates
hormonal involvement.
Your X-Ray
paranasal sinuses indicates deviation of bony part to left along with an spur on the same side. This deviation and spur had given rise to nasal obstruction and ultimately giving rise to haziness in both maxillary sinuses. Since your other than max sinuses are normal thus it is less likely to give headache directly but even though there are two other conditions may cause headache-
(1) Deviated septum along with spur if touches the middle turbinate may give rise to
severe headache.
(2) Deviated septum causes nasal obstruction and thus stuffy and blocked nose give rise to recurrent
sore throat which in turn blocks Eustachian tube thus ear gets blocked and hurts, this may be severe enough to involve head.
Regarding treatment you may take antibiotics, anti inflammatory, decongestant , Mucolytics and steam inhalation.
