Hi
Testicular pain can be due to inguinal hernia which you told you ruled out.It can also be due to varicocele or epididimitis or hydrocele or other lesions inside.Even it can be due to kidney stones. You should rule out all these with the help of an experienced general surgeon. The further treatment should be decided by the surgeon whether you can be carried on by pain medication or surgical treatment.
hope this may help you
thanks

General & Family Physician Dr. Shafi Ullah Khan's  Response
hello there!
Thank you for asking, At the age of 64 testicular pain for 4 months with prostate assessment cleared and no hernia involvement only options of epididymitis or orchitis are left. The indication for that would be if you lift the testicles with your hand your the pain will vanish,
Any how you need an examination by a general surgeon to assess the situation and look for any positive finding , Answering on basis of just pain would be hitting rock with head,.
Seek a G surgeon help.

Good Luck
S Khan
