Have Type 2 Diabetes. Taken Metformin. Done Blood Test. Should I Be Concerned?

I am a type 2 diabetic and have known about it for just over a year. A1c was 13 originally but with Metformin and low carb I have it down to 5.8. I just had a blood test showing microalbumin,ur,qn at 30.7, creatine,unine at 215 and Microal/creatine at 14.2. How concerned should I be? Each of these results are quite a bit larger than 6 months ago. I am taking some supplements but the only new one is Cardioviva. I will also be discussing this with my Kaisar doctor but just curious of what you think? I also have been eatting some sugar and junk carbs lately ( 3-4 weeks ) which I haven't been doing in over a year.

Hello,
Going by your current reports your albumin in urine is 30.7 while below 30 is normal and microalbuminaria is said to be present if the values are between 30-300.
Creatinine levels and albumin/ creatinine ratio are also normal.( ACR )
Normal urine creatinine levels are 14-26 mg per kg of body in men.
Similarly upper normal limit of ACR is 25.
In my opinion your reports are normal at the moment however you should be cautious in keeping your blood sugar levels under meticulous control.
To avoid future diabetic nephropathy you should keep your blood pressure under proper control and must be put on small doses of ACE INHIBITORS OR ARB's even if BP is normal.
Microalbuminaria is potential reversible state and if allowed to worsen,it leads to overt proteinuria and diabetic nephropathy.
There doesn't appear to be any sign of microalbuminaria however you should be cautious.
Get a 24 hour urine albumin esitimation done for proper diagnosis.
Kindly don't spoil your good sugar levels by resorting to unnecessary sugar and junk food intake.
Keep a strict watch on your calorie intake and do regular exercises.

MICROALBUMINURIA INDICATES THE ONSET OF MICROVASCULAR COMPLICATIONS, IT SHOULD BE GIVEN ATTENTION TO DELAY THE PROGRESSION OF IT.

SO START A LOW DOSE ACE INHIBITORS, AFTER CONSULTING YOUR DIABETOLOGIST. SECONDLY CONTROL YOUR BLOOD SUGARS WELL, IF YOU ARE DOING GOOD ENOUGH WITH METFORMIN THEN CONTINUE IT. AFTER REVIEWING YOUR GLUCOSE PROFILE READINGS, THEN YOU CAN DECIDE THE NEXT ADD ON THERAPY.

Kindly tell us al the medication you are taking now. Strict sugar level control is recommended but sudden decrease in HbA1c level from 13 to 5.8 is not acceptable as it is very strict control. And your renal function test shows problem in kidney. Need evaluation for it whether due to diabetes or due to other cause. Get done your complete hemogram, urine routine, blood sugar and lipid profile and consult your doctor for examination and further management.

Hello!
Thanks for query.
If your HbA1C is below 6.5 or 7 you need not to change your medicines.You can continue with metformin and diabetic diet.Your microalbuminurea can be treated with ACE inhibitors.

Continue with same treatment and get repeat testing for Blood sugar levels,renal function test and urine for albumin.

Consult with your physician for prescription of medicines if reports found abnormal.

your blood glucose control seems to be very good. no microalbuminuria, and creatinine also seems to be in normal limit. about your concern on increased value from previous, as long as it is in normal limit, you need not bother
