Have Pain In Ab, Pelvic Area. Painful When Touching, It Is Tender. Feel Thin Lump. Swollen. Suggestions?

Last week I began having pain on my lower right ab, right above the pelvic area. The pain was more when I touched the area, it was very tender. Now, the tenderness is not as bad but I feel a thin sized lump going from my lower right abdominal area up toward my belly button. Somewhat tender to touch. Almost like it is swollen. Any idea of what can cause this. No other symptoms. Except I've never felt that and the tenderness to touch is not normal.

Thanks for the query to H.C.M.. Forum.
If I were your treating doctor for this case of severe pain in the right lower abdomen , I would come up with these possibilities , these includes.
1 The first possibility is of appendicitis ,diagnosis can be confirmed by physical examination by a surgeon?physician and ultrasound of lower part of abdomen .
Get in blood for C B C also. Usually appendicitis associated with pain and vomiting.
After acute attack , usually , if not treated it turn into chronic attack , known as appendicular lump as you yourself feeling .
2 Ovarian cyst , diagnosis can be confirmed by ultrasound of abdomen with emphasis over right ovary.
For this consult a gynecologist and get his treatment and opinion.
3 Salpingitis i.e . inflammation of right fallopian tube , diagnosis will be assessed by gynecologist after tubal ultrasound.
Sometimes large intestine may be the reason so get in ultrasound.
So the the best way is consult a surgeon and get his opinion.

Hi ! I am Dr Shareef, a general surgeon answering your query.

From your symptoms as you have described, I must tell you that you possibly have neglected your health to a great extent. Possibly, it was an inflamed intraabdominal organ like the gall bladder (cholecystitis) or an appendicitis. Because of the delay in treatment, the omentum and possibly the intestines have got adhered to the inflamed organ forming a mass, which is nothing but one of the nature's defense mechanism to avoid spreading of the infection. This being an inflamed mass may irritate the parietal peritoneum (local peritonitis) and give rise to such pain and tenderness in the respective area.

I would advise you for an ultrasound of whole abdomen to have a rough idea of the organ involved, apart from routine blood tests including a blood sugar test.

If I would have been your treating doctor, I would admit you in a hospital, and start a broad spectrum antibiotic with IV fluids for your nutrition, and try to control this infection in a conservative way and plan an elective surgery ( interval appendicectomy/cholecystectomy) at a later date preferably after 6 weeks.However, if the symptoms worsen, I will keep the option open for an emergency surgery to avoid further complication.

Under these circumstances I would advise you to report immediately at an ER of any nearest hospital and get yourself examined and investigated for further definitive treatment. Good luck and thanks.

Without actually examining your belly, it is very hard to say. This possibilites might include:

1. An ovarian cyst
2. A hernia
3. Related to your bowels (appendicitis, constipation)
4. Abdominal wall strain
5. A pregnancy in the right tube

So take a pregnancy test now and go in to your doctor to be seen.

Thanks for writing in to us.

Any possible lump in the abdomen must be seen by and clinically confirmed by a surgeon. The presence of tenderness make is more of an acute medical condition.

It can be an infection just below the skin involving the anterior abdomen muscles in most cases. It can also be something inside the abdominal cavity like bowel origin. An ultrasound scan will help in confirming the lump and what exactly is happening in the area of tenderness.

Your doctor will correlate his clinical findings with that of ultrasound scan and treat you accordingly.

From history it seems that you might be having acute appendicitis giving rise to right lower abdomen and lump in that area.
Consult surgeon and get examined.
On physical examination one will have clear idea about your problem.
