Have Pacemaker. Trouble Breathing At Night. Reason?

My brother in law is 92 years old he has had 2 pacemaker done he has had the second one in for about 3 or 4 years he is complaining about when he goes to bed at night he is having trouble breathing which causes him to get up and sat up right in the chair all night went to the heart doctor and several other doctor they could not find anything wrong what could the problem be he has had a couple of episodes latest one was within a week.

This description is typical of left heart failure.
It is probably not related to the pacemaker.
He has to be investigated for a cause (example - silent heart attack) / he needs lab tests, ECHOcardiography and so on.
Discuss with his doctor..
