Hello user,
Its good that you have found a matching donor of organ for the transplant procedure.
As you have not specified any organ for transplant so I can not specifically explain the procedure. In general speaking, the donor organ n removed from donor, and in adjacent operation theater the receiving persons abdomen is opened and the matching organ is transplanted and its blood vessels are anatomized.
The receiver has to take certain immunosuppresive drugs, to prevent rejection of the organ which is being transplanted for few months or years or maybe lifetime as advised by the surgeon.

hope my info helped you
