Could The Renal Problems Be Due To The RA Or The Medications Causing Edema Of Legs?

My brother is 65 and has RA. recently he noticed a weight gain of 15 lbs in 41 days due to edema of his legs and feet. He has been diagnosed with pericarditis. He was put on aspirin and Lasix with some results. He is still short of breath when climbing stairs. He also has proteinuria. His Dr. is now telling his he is in renal failure. His bun and creatinine were both normal, I don t think they have done any special chems. at this time..(SPEP ,UPEP etc.) His joint pain is being controlled by Plaquenil. What do you think..is the RA causing renal problems or is the medication. What tests would you do?? He is NOT diabetic.

Medicines can cause renal injuries but without change in renal function parameters difficult to interpret. May need some more tests. RA can cause pericarditis. Response to treatment confirms. Grade of proteinuria is more informative. Low albumin won't give much information in swellings or oedema but only lower limbs Albumin can be of some help. Symptoms may need long therapy with Diuretics. watch for renal functions. infections also common in RA. Heart involvement is common if such symptoms so cardiology consultation must.
