Chronic pancreatitis, intense pain subsides with morphine that causes constipation. What can be done to relieve this side effect?

I have Chronic Pancreatitus. The only thing that helps the pain is morphine . ( I go to a pancreatic clinic monthly at u-mass hospital in Ma.) To be comfortable I should be taking morphine during the day and at night but because of constipation I sometimes skip my morning dose. I had part of my colon removed and the rest are very sensitive so I have to stay low fiber . What else can I do about the constipation? I currently take Amitiza twice daily as well as 3 softners at night.

Hello,

Sorry to say, but drug-induced constipation needs medicine change as the only option in your case as you have to consume low fiber diet.

There are best alternatives available as drotaverine and others for pain relief of pancreatitis without constipation and the same should be discussed with your treating gastroenterologist.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Bhagyesh V. Patel, General Surgeon

I have been taking Percocet for a decade for chronic pancreatitis. I have stopped taking the percot since sept16. They have given me 4 mg of suboxcen. Although the pain level is under control. I would say 75 percent of the pain is relieved. But there are some days that the pain is intense. I do not take percent for fun or a high. I take it after having seen drs is mass ,Florida, and California. I have seen gastroenterologist and been referred to pain clinics which all say they can t cure this chronic pancreatic and the final result each time is to mama get my pain with Percocet. I took it into my own hands went to rehab for7days. And have been actively participating in a five hour a day. M-f. I am in a lot of pain right now. I took 4mg of suboxin and I found a 5 milligram percot and I really am debating whether to just take it to relieve this pain. I read side effects and seems like a low dose should be ok. Can I take this pill to get past this pain

I am a 36 years old female with more than ten years history of chronic bloating and chronic constipation . Different tests showed no abnormalities in my bowels, so Gastro suspected IBS-C and just recommended to manage with diet and laxative (Milk of Mg, Miralax, etc). I tried drugs like zelnorm and amitiza w/o success. Lactulose made bloating even worst. Also, off-label use of a drug similar to colchicine , which name I can t remember. This one seemed to work but stopped it because I was afraid of its teratogenic effects. High-fiber diet does not make a difference. Eighteen months ago, I gave birth, and bloating and constipation were a little better after delivery. About a month ago, constipation and bloating have been terrible again, and started to have symptoms like an UTI. The OB-GYN diagnosed me with Interstitial cystitis . I am currently, taking Elmiron, Hydroxyzine , IC-friendly diet, and having a series of bladder instillations. I feel better regarding my IC symptoms, but noticed that whenever the constipation and bloating are bad, IC flares up. Currently, I am having liquid bowel movements (with Milk of Mg) and noticed my breath and stool smell like a chemical substance. Bloating is still there, even if I have a bowel movement, and aggravates along the day. I am following the IC-friendly diet, but there are some things that are IC friendly (ie wheat), that aggravates my GI problem. I read about a low FODMAP diet, but I would like to consult a Gastro about these because I am feeling like doing both diets is very limiting. Also, I would like to have things under control as I am thinking of getting pregnant again. I am worry about nutrition, and gaining weight during pregnancy. I will see a gastro in a couple of weeks, and want to bring up this IC diagnosis, and possibly evaluate for SIBO or celiac. No Dr has ever mentioned any of these conditions, but I read that IBS-C and these have overlapping symptoms and you can end up being misdiagnosed. Can you please provide an opinion? I need some help in the meantime.

dear doctor, My mother has been diagnosed with Henoch Schonlein Purpura. Three weeks later her kidney function level is declining creatinine levels 293. She is sleepy, complaining of nausea, dizziness, vomiting, swollen stomach (even without eating), stomach pain, bloating, constipation, difficulty in bowel movement. She has heamogloninaemia and hypertension. Could she have twisted bowel syndrome ? She is going down hill fast. What is the prognosis?
