It is very uncommon to have migraine
at 5 year old male child, but it can happen at earlier age also. As he is also having nasal bleed it is must to get any ENT pathology ruled out especially Chronic Sinusitis
which may present with headache.
Also he should be shown to some good Neurologist
, and get his EEG done because frequent headache in a young child may be a clinical manifestation of epilepsy
Secondly his blood investigations should be done, any evidence of Hyperlipedemia to be ruled out as micro emboli may lodge in blood vessels and can cause headache.
