Child Have Fever, Diagnosed As Typhoid Hepatomegaly. Have Family Medical History. What To Do?

Good morning doctor,
Master. Varun 3 year old boy suffering with evening rise of temperature since one week, one episode of febrile fits, the temperature vary from 103'F - 105' F. Now they have diagnosed with post typhoid Hepatomegaly. 3 months back he was treated with typhoid fever. His father having history of Convulsion and he was under treatment for many years. Often this child is getting respiratory tract infection and in their family history Grand father had TB. what all the investigations he has to undergo, and what type of treatment will be appropriate for this 3 year old child doctor.

Thank you for your query.

You have three queries which I shall try to address:
1. Hepatomegaly.
2. Febrile convulsions.
3. Repeated upper respiratory tract infection.

In typhoid fever, liver does enlarge and it takes time to return to normal. You should not worry about it but keep getting him checked frequently.

Febrile convulsions can occur whenever the temperature rises very fast. It is not necessary that it shall recur. However with a strong family history one needs to be cautious and in case recurrence occurs medication will have to be considered.

At this age repeated upper respiratory tract infection are very common as the child is building his immunity. Ensure that the child has a good nutritious diet. It will be advisable to get him investigated to rule out other causes of infection.
