Child has rash on bottom. Diagnosed as strep and gave antibiotic. No fever. Solution?

my 6 year old son has a rash on his bottom. I took him to the Dr about 2 week ago and they diagnose him with strep and prescribed an antibiotic. I worked for a week but now it is back! could it be something else? No fever or other symptoms. I have used diaper cream with zinc the last two days, still not better. Any ideas?

any history of trauma
or worms infestation
is this associated with diarrhea or nor
