Chest Pain. Colonoscopy And Endoscopy And All Is Normal Except For A Grade A Esophagitis. Any Suggestions?

Dear Dr. My husband since last month has experienced varying symptoms
I night he woke up with a severe crushing pain in the chest
then the pain was felt deep at the lower part of the rib cage as a pinching pain that comes before eating and changes to a muscle cramp after eating. We did stress test, colonoscopy and endoscopy and all is normal except for a grade A esophagitis. Today he has a tingling sensation on the chest back and arm.

Was an ECHOcardiogram done?
CT chest and cervical spine may be done to exclude lung problem
