can I safely take promethazine-DM 2 tsp every 4-6 hrs, with Alka-Seltzer plus multi symptom cold formula (containing dextromethorphan hydrobromide 10 mg and phenylephrine bitartrate 7.8 mg and aspirin 325 mg)Nite time Alka Seltzer contains same but 500 mg aspirin + doxylamine succinate 6.25 mg

It interacts, but it is likely within a safety range. The dextromathorphan is in both promethazine DM and multi-system relief. It obviously adds up. Furthermore, DM is a potent inhibitor of liver metabolism including probably promethazine metabolism. Both of these will double. But that is still likely to be within safe dosing unless there are other features like cirrhosis of the liver or very small size.
