Birth Asphyxia. Requested CPK, Abnormal. What Can We Expect?

HI, I AM A DOCTOR GETTING MY TRAINING IN PAEDIATRICS. I HAVE A PATIENT ADMITTED IN MY NURSERY WITH BIRTH ASPHYXIA. HE IS UNABLE TO CLOSE HIS MOUTH(showing degree of hypotonia). i requested his CPK levels. The levels at 72 hrs of life are 178 and CK-MB 76. kindly guide me how to proceed. and are these levels significant? There is history of sib deaths in family with no issue alive and 1 died at 3 months of age with no known cause. kindly help me in management. thanx.

Hi,
Cpk of 178 is not high for asphyxia.
However ck mb should be less than 2% of total cpk to be normal.
If high it indicates cardiac affection from asphyxia.
For SIB death you have to do compete metabolic workup ,ammonia,blood gas,blood amino acid profile,carnitine,etc.
