Abdominal Pain At The Time Of Bowel Movement. Taking Metrogyl. Advice?

Sir,now I am 55. I have been suffering from abdominal crampy pain at the time of bowel movement with mucoid stool and flatulence since 1985. Stool usually two times a day. Findings of colonoscopy (1995) is NSC. In 2006 it was normal with small haemaroids. Doctors,diogonise it as likely IBS. About a year back I was advised to take Rabeprazole 20, Ibscim and Rcifax. Now no pain during bowel movement regularly, but not at all relieved of flatulence. During the last two years flatulence has increased. Antacid like Somprazol or Omeprazol have got no effect. Presently though there is no regular pain during bowel movement, I suffer severe abdominal crampy pain if I take outside food. Metrogyl is effective during the course of taking the medicine except acidity, but within a week after completing the course crampy pain relapses. Presently on medication of Menoctyl. Please tell me the side effect of these medicines. As there is no treatment of IBS should I stop taking medicine and bear with these sufferings. Solicit your valuable advice to be relieved of my sufferings.

Hello! Thanks for putting your query in HCM. I am a Gastroenterologist (DM).

Few things to know about IBS:
1. Is a chronic disease which has relapsing and remitting course
2. It will never harm you even if it will persist for whole life
3. Stress, life events, and anxiety leads to aggravation of symptoms
4. Intestine becomes hypersensitive so even discomfort is perceived as pain
5. Increase gas production leading to abdominal fullness and bloating
6. No alarming features like weight loss, gastrointestinal bleeding, or fever

Menoctyl contains otilonium bromide and it's side effects are blurring of vision, constipation, dry mouth, drowsiness, urinary hesitancy and sometimes muscle weakness.

I will like to advise you following:
1.See whether milk or milk products worsen your symptoms, if yes, please avoid them
2. Take Tablet gasex by Himalaya. It helps in relieving gaseous symptoms like bloating, flatulence.
3.Instead of Tablet Menoctyl take Tab mebeverine which has less of the above mentioned symptoms and will help in relieving your crampy pain. If pain is not relieved with this then take antispasmodic like drotavarine.
4. Food from outside leads to worsening of your symptoms which is relieved with antibiotics suggest that you may get infection from outside food. I will advise you to avoid outside food if feasible. And de-worm once in every six month with tablet Albendazole
5. If possible get you stool examination for reducing sugars. If it is there then see for any indigestible sugar you are taking and avoid that.
6.Avoid fatty meals
7. Don't lie down soon after meals for at least half hour
8.Take small and frequent meals, instead of full meals at a time
9. If you are overweight or obese do weight reduction with dieting and exercise. Do regular aerobic exercise otherwise also for at least 30minutes per day
10. Avoid smoking if you do
11.Take some prokinetic agent like domperidone regularly and Tablet rabeprazole sos for acidity
12. Avoid carbonated drinks, wine or beer
13. take black tea and add cardamom and ginger to it and take them with meals

MOREOVER THE MOST IMPORTANT THING TO REMEMBER WHICH I AM REPEATING IS THAT IBS NEVER CAUSE ANY HARM EVEN IF IT PERSIST FOR WHOLE LIFE

You are suffering from problem of Dysentery & IBS.

* Side Effects of Menoctyl =
Blurred vision,
drowsiness,
constipation,
dry mouth,
muscle weakness.

* Side Effects of Metrogyl =
Dizziness, headache, diarrhea, nausea, stomach pain, loss of appetite, constipation, changes in taste, and dry mouth may occur. If any of these effects persist or worsen, notify your doctor or pharmacist promptly.

This drug may cause urine to darken in color. This is harmless.

You should consult Proper Surgeon or Physician. Do appropriate test as advice by Doctor.

Advising You , Not to eat Wheat containing food,Fast food, Spicy food etc.
