6 year child, reoccurring UTI. Negative kidney ultrasounds, scans. Taking antibiotics. Treatment?

My daughter 6 years old has repeated urinary tract infections. We have done kidney ultrasounds, kidney scan and the scan where the catheter is injected through the vagina. All these tests have come back negative, and she passes stool usually every day or every other day. Once she comes off the antibiotic the infection comes back within 2-3 days. This has been happening for almost two years. What else can we do?

Thank you for your query on Healthcare Magic.

Frequent urine infections are common in girls in comparison to boys because of short urethra.

As you have ruled out underlying urinary structural abnormalities by investigations, still the cause for recurrent UTI is unknown.

I advise you to do the following :

Ask her to drink plenty of water to make a good urinary stream.

Ask her not to with hold urine for frequently for prolonged times.

While cleaning stool she should clean from front to back as to avoid the contamination of genital region by the stool.

Any associated constipation has to be treated effectively with dietary fiber rich food and medication.

Discuss with your Pediatrician for further option like keeping the child on long term antibiotic prophylaxis for prevention of urine infections.

Thanks for writing to health care magic.
You have ruled out structural abnormalities and reflux of urine from all those investigations.

But is this urine infection confirmed from laboratory reports of urine examination all the time?

What was the urine culture and sensitivity report telling you.

Hope you got the complete set of investigations like MCU, DMSA, DTPA, etc.

Was your child's blood pressure taken any time?

Take basic precautions like

1. Ask the child to void urine frequently.
2. Ask to double void urine. Once she completes passing urine ask her to go and void urine once again.
3. Give her plenty of water to drink.
4. Avoid constipation by giving lot of fruits and green leafy vegetables.
5. Do not clean the anal region from front.

Continue to ask further queries as required.
