Brief Answer:
A nasal endoscopy, sleep study and a plain CT PNS are suggested.

Detailed Answer:
Hi,

Thank you for your query.

1. Chronic nasal congestion, turbinate hypertrophy, DNS (Deviated Nasal Septum), acid reflux and SDB (Sleep Disordered Breathing) are common causes of these symptoms. Since cardiac and respiratory causes have been ruled out, you can get the turbinate hypertrophy treated with good results.

2. I recommend that you get a plain CT PNS (Para-Nasal Sinuses) done pre and post de-congestion with 0.1% Xylometazoline nasal decongestant drops. The procedure is as follows. After the first scan, use 5 to 8 drops of 0.1% Xylometazoline nasal decongestant drops in both sides of the nose. Wait 15 minutes and rescan. The radiation settings can be reduce by half. Print these images side by side and compare and co-relate with any subjective improvement of your nasal congestion. A nasal endoscopy is also required.

3. Nasal turbinate reduction with radio-frequency or coblation technology is a fairly simple, quick and safe procedure given your history.

4. A sleep study will also help. Weight reduction and treatment of acid reflux will prevent further increase in your symptoms.

5. Upload the endoscopy, sleep study report and scan images for either here or directly at bit.ly/Dr-Sumit-Bhatti

I hope that I have answered your query. If you have any further questions, I will be available to answer them.

Regards.

Detailed Answer:
Hi,

Thank you for following up

1. You can start with a 15 day course of Tab. Montair-FX once a day, Ezicas AZ nasal spray once a day, Cap Curcon once a day, steam inhalation once a day.

2. In cases with turbinate hypertrophy, it is expected that in some cases these medications will have limited benefit.

3. In such cases, investigations are done early.

I hope that I have answered your query. If you have any further questions, I will be available to answer them.

Regards.
