Brief Answer:
This needs medical attention

Detailed Answer:
Hello, I'm Dr. Branch, thanks for using Ask a Doctor.

The wound that you have shown needs to have medical attention. The first thing that should be determined is the cause of the wound. This may be a venous or an ischemic ulcer, or even could be cancerous, which can be determined by a physical examination and possibly a biopsy of the wound. It also looks like you may need surgical debridement of the wound that may be stopping it from healing. I'm not sure how long you've had this wound or if a doctor has seen it yet, but if you have not is that a doctor see it yet it certainly needs help if it is to heal. Just putting antibiotic ointment will not be enough for this one to heal in my opinion.

I hope that helps, please let me know if you have any other questions and I would be glad to discuss it with you further. But ultimately believe you will need medical evaluation.
