Based on the history you have shared, one thing to know is that it's a progressive disease and if treatment sent adequate it may progress rapidly so repeated and persistent reviews with a neurologist are strictly advisable.

The current symptoms can be due to something as slowly with dopamine supplementation from outside the wearing off phenomenon and on and off phenomenon are observed.

Also some efficacy may be lost. Stiffening of body is also a recognized side effects.

There are other set of medications which can be used as adjunct to sinemet like dopamine receptor agonists like ropinorol and Monoamine oxidase inhibitors or selegelline and amantadine etc as well as using anticholinergic drugs which can cure the stiffened muscles or dystonic muscle contractions. But all of these need to be started with close follow up and hence repeated sessions with a neurologist may be needed.
