Brief Answer:
Colonoscopy and Proctoscopic needs to be done.

Detailed Answer:
Hello and thanks for choosing 'Ask a Doctor' service for your query,
As per the history bleeding is there from lower parts (colon) as it is bright red in color.Pain while defecating along with blood in stools could be a sign of carcinoma.
Have you ever had a colonoscopy done?
If not it is recommended that you should have a colonoscopy done. Even if you have had this test in the last 5 years it might be recommended to repeat it as you are having this bleeding if the stool tests confirm you are really bleeding.

So test required are-

1)you will need a colonoscopy to find the cause of it.

2)if you have bright red blood while defecating (passing stools) this might also be due to haemorrhoids which are mostly due to constipation so in meantime please make sure you take extra fibre with diet and drink plenty of water.Proctoscopic examination of anal canal would also be done.

3 If you had any massive rectal bleeding than an urgent visit to a doctor is needed.

I hope this is helpful and answers your questions but please feel free to reply in case any more clarification is needed on this.

Kind regards,

Thank you for the response. I have regade and every time I defecate I feel getting hurt because I have problems with constipation. After that I notice red blood. I have done the colonoscopy and told me that I have anal regade . I will go tomorrow to the doctor but I am afraid that it might be a sign of cancer.

I am sorry maybe I used the wrong term. I don't have the report because I have done it more than 6 years ago. I meant fissure. The doctors told me that due to constipation, I have a cut and this cause the problem. This was told me years ago I though it was the same problem happening again now

Brief Answer:
Surgery should have been done already. No use of wait and watch.

Detailed Answer:
Hi there,
If colonoscopy was normal it can't be cancer.
As per fissure is concerned that is because of constipation.
Our anal and rectal canal are lined by soft layer of mucosal cells. When ever a person is severely constipated he strains a lot while passing stools. This over straining causes the soft layer of cells to erode away.If not treated properly slowly and slowly it leads to formation of full fledged wound called anal fissure.
Treatment is surgery,
Surgical cure now days have a high success rate.
Till the time surgery is done use oint. neosporin over the area of itching.
Try to keep anal area dry,
Go to surgeon and get yourself operated.
Thanks.
