Hi,
Thanks four your query to XXXXXXX

The possibilities are
1. Varicocele- engorged veins of the testes.
2. Hernia- descent of the stomach contents into the scrotum.
3. Hydrocele- Fluid collection around the testes.
4. Twist of the spermatic cord ( in which case the pain will be very severe and hence unlikely).
5. Last but not the least - testicular tumour.

You need focused history, physical examination, and possibly ultrasound scan of the abdomen and testes.
The treatment will vary based on the cause found.
Till that time, you can use little tight underwear to support the scrotum.
Hope this helps, kindly get back for doubts if any.
