Question: Hello Doctor, We recently got married on Dec 6th 2013, and have been undergoing intercourse daily with the intension to be blessed with baby as soon a possible. Here are few details of both of us: Age: I'm 36 years old and my wife is 35.5 years old. About her menses cycle: - she has average of 35 to 37 days, and keeps on fluctuating. Here is a record of past six month’s menses cycle for your easy reference. Date Menses-Cycle 25-05-2013 29-06-2013 35 12-08-2013 44 14-09-2013 33 19-10-2013 35 25-11-2013 37 Medical history: I have slight hypertension, of 110/130 and consuming prescribed tab Concor-5mg once in every morning. No allergy for any drugs experienced. Can we expect or keep hope this time if not, could you please explain which is the best period for intercourse that can give positive result of conceive. Also, we would like to have your recommendation about the intact of healthy diet and any other health care guidance related to get conceived. Looking for your response. Thank You, XXXX

Detailed Answer: Hello, Thanks for writing to us on XXXXXXX According to given information, your wife's monthly period cycle is quite irregular and it may occur due to mental stress or emotional upset, hormonal imbalance, Poly Cystic Ovarian Disease, over weight or rapid loss, pelvic infection, anemia, irregular diet habit etc. Here, I suggest to undergo relevant investigations like pelvic ultrasound scan, hormone test, thyroid profile, LH/FSH, blood Hb% etc to pinpoint the diagnosis and rule out underling pathology if any. You can consult with GYNECOLOGIST for proper guidance and complete prescription. For successful conception, both of you need to follow these suggestions: 1) First, she needs a regular cycle with proper ovulation. Progesterone pill (D16 to D25) and other supportive treatment may be required in this case to regularize her monthly cycle. 2) For proper ovulation, she may need to take ovulation induction drugs like Clomiphene from D2 of her cycle. Serial follicular study (D12 to D16) is useful to monitor her ovulation or she can use ovulation prediction kit (LH kit) to track down ovulation time in the middle of each cycle. 3) You have to do regular unprotected sex close to her ovulation day (14 days prior to next due date) or (11th to 19th days of each regular 28 to 30 days cycle) to maximize the chance of pregnancy. 4) Both of you must take a healthy balanced diet with more fresh fruits and vegetables.
